A Case of Chronic Pancreatitis with Small Pseudocyst in Childhood






Abstract
A 8-year-old man presented with recurrent epigastric pain. He was admitted to the pediatric ward with chief complaints of servere epigastric pain at postprandial and poor appetite. Serum amylase at preoperative state were 640-800 U/dl. On the
abdominal
ultrasound and abdominal CT scan, there were pancreatic psuedocyst, peripancreatic fluid collection, focal necrotic portion in the tail of panceas and abdominal ascites, but no any of the demonstrable causes was found.
On the 29th hospital day, an exploratory laparotomy was performed. The tail of pancreas with hard consistence and focal fat necrosis was found. The body of pancreas with 1.5 cm sized pseudocyst was found. Distal pancreatectomy and Roux-en-Y
cystojejunostomy performed. Serum amylase at postoperative state were 100 U/dl.
Pancreatic pseudocyst and focal necrosis are virtually pathognomonic of chronic pancreatitis. We report this case and review related literatures briefly.
